The Fire department keeps records for medical emergencies we respond to. Medical information is confidential.

Need to know

  • No cost
  • You can request your own records
  • You cannot request someone else’s records unless you have power of attorney or are their lawyer with a signed waiver
  • Turnaround time is typically 2-10 days

Before you start

There are 2 types of records we can provide:

  • Medical incident report
  • Electronic health record

These reports typically include:

  • Patient information
  • Medical assessment
  • Treatment provided
  • Narrative description of what occurred
  • Insurance information

Be as specific as possible in your request, including any dates if you know them. This helps us find exactly what you are looking for.

Steps

  1. Call (510) 420-3030 to make a medical record request.
  2. We will respond within 2-10 business days to confirm if we have the records or if we need more information from you.

To get your records, you’ll need to show your ID to prove your identity.

You can get your records by:

  • Fax
  • Pick them up in-person
  • By mail
